In 2026, our hospital will conduct a “Social Responsibility and Sustainable Development Report.”
To identify and develop key issues for the hospital, we are distributing this questionnaire to relevant stakeholders, including Chung Shan Medical University Hospital employees, supervisory teams (board members, Chung Shan Medical University supervisors), patients and their families, regulatory authorities (Taichung City Health Bureau, Taiwan Joint Commission on Hospital Accreditation, health centers, National Health Insurance Administration), suppliers and contractors (partners), and other related units. This will help us understand the topics of concern and impact from various perspectives.
This survey is conducted anonymously, and the collected data will be used solely for analyzing external concerns for our report. It will not be used for any other purpose. We sincerely appreciate your time in completing this questionnaire.
